Each year, Montachusett Home Care Corporation is able to help over 2000 elders remain at home through the State Home Care Program. Funded by the Executive Office of Elder Affairs, this program is designed to help low to middle income senior citizens maintain their independence by offering a wide array of service options that can address a variety of problems.
A nurse or a geriatric case manager will respond to referrals by visiting with seniors in their homes to assess their needs, determine eligibility, and discuss which service options may best assist them. Once an individualized plan of care is developed, the case manager will implement it and continue to monitor the quality and effectiveness of the services.
Who is eligible for State Home Care services?
- Persons aged 60 or older
- Who meet an income criteria:

- Who have functional impairments and critical unmet needs
as determined by a comprehensive assessment by a case manager and /or nurse.

What are the services available?
By contract with closely monitored Service Providers:
- Homemaking: housework, laundry and shopping.
- Personal Care: hands on assistance with bathing, grooming, walking.
- Minor Housing Adaptations.
- Chore-work: heavier household tasks.
- Social and Day Health programs
- Home-delivered meals
- Dementia Day Care
- Home Health Services: RN, Home health aide
- Supportive Home Care Aides
- Medical Transportation
- Low Vision Rehabilitation Counseling
- Alzheimer’s Safe Return Program Enrollment
- Alzheimer’s Caregiver Counseling
- Short-Term Placements in Adult Family Care, Rest Homes, or Nursing Homes
- Personal Emergency Response Systems

What are the costs to participants?
Most of the cost of services is borne by the Commonwealth through the Executive Office of Elder Affairs. Fees are assessed on a sliding scale. Some clients are asked for a voluntary donation of $5.00. No one is ever charged more than the maximum of $105.00 per month for all services.
